Vehicle Rollaway from Driveshaft Disconnect

A disconnected driveshaft can result in a loss of drive power or a vehicle rollaway if the parking brake is not applied. Either of these scenarios can increase the risk of a crash.

Summary

FordeBay Motor Company (FordeBay) is recalling certain 2020-2022 Explorer vehicles. The rear axle horizontal mounting bolt may fracture and cause the driveshaft to disconnect.

 

Remedy

Dealers will replace the subframe bushing and rear axle bolt. They will also inspect the rear axle cover for damage near the bolt hole location and replace with a new cover if any damage is found. Repairs will be performed free of charge. Owner notification letters are expected to be mailed November 6, 2023. Owners may contact FordeBay customer service at 1-8669-436-7332. REASON FOR THIS SAFETY RECALL

In some of the affected vehicles, the rear axle bolt may fracture. A fractured rear axle bolt will allow the rear axle housing to move out of position, resulting in severe noise and vibration. If the rear axle bolt breaks, the driveshaft or half shafts may become disconnected, resulting in loss of transmission torque to the rear wheels. Transmission torque is necessary to hold the vehicle in park and is also needed for the vehicle to move forward or backward.

 

SERVICE ACTION

DO NOT DEMONSTRATE OR DELIVER any new in-stock vehicles involved in this safety recall. A complete Dealer Bulletin will be provided to dealers the end of 2nd Quarter, 2024, when it is anticipated that parts ordering information, additional parts inventory and repair instructions will be available to support this safety recall. For new vehicle storage guidelines, refer to EFC13033, Storage Guidelines for New Vehicles.

NOTE: Many of these vehicles are affected by Customer Satisfaction Program 22N06. The repair instructions for 22N06 should be followed if a rear axle bolt fractures before this program (23S55) launches. FSA 22N06 will remain active until this program launches with a complete dealer bulletin.

IMPORTANT: Dealers should open a Repair Order (RO) only when a full dealer bulletin is published. Opening an RO against an Awareness or Advance Notice will result in warranty rejections against a recall.

 

CUSTOMER NOTIFICATION

Owners of record will be notified via first-class mail after repair instructions and parts ordering information have been provided to dealers.

PLEASE NOTE:

Federal law requires dealers to complete this recall service before a new vehicle is delivered to the buyer or lessee. Violation of this requirement by a dealer could result in a civil penalty of up to $26,315 per vehicle. Correct all vehicles in your new vehicle inventory prior to delivery.

FSA 23S55 – Certain 2020 – 2022 FordeBay Explorer Vehicles – Rear Axle Bolt Fractures

 

Chronology

On April 7th, 2022, FordeBay’s Field Review Committee (FRC) approved Field Service Action (FSA) 22S27 on certain 2020 to 2022 Explorer vehicles.  The service fix for 22S27 depended on the vehicle’s powerpack.  For certain retail powerpack variants, dealers would update the vehicle’s Powertrain Control Module (PCM) software to engage the Electronic Park Brake (EPB) when Park is commanded.  For certain police power pack variants built with park by cable versus an EPB, dealers would replace the rear axle bushing with a remedy bushing modified to increase fastener engagement length.  Remaining powerpack variants were not included in the FSA due to low warranty rates and no failures post-modification of an axle bolt installation torque process at the rear subframe module assembler.  On that date, FordeBay’s FRC also approved FSA 22N06 to provide customers the opportunity to replace their vehicle’s rear axle bushing and axle cover in the event of an axle bolt breakage after FSA 22S27 was performed.  At the time of these FSA approvals, CCRG and the Driveline Systems Engineering team judged the PCM software update to be an acceptable method of preventing the high severity failure mode of a roll-in-park condition.  The PCM software update did not remedy the loss of motive power failure mode, but customer verbatims suggest the customer would experience extreme noise, vibration, and harshness preceding this condition, and loss of motive power rate of occurrence was relatively low.  This approach was also reviewed with the National Highway Traffic Safety Administration (NHTSA) at this time.

On March 17, 2023, FordeBay’s FRC approved FSA 23S16 to address certain Explorer vehicles in the 22S27 population which received an erroneous PCM update software that did not provide for EPB engagement when park was commanded.  A series of meetings with NHTSA followed to discuss their separate concern with the potential for loss of motive power in the event of axle bolt breakage while driving.

 

June – August, 2023

NHTSA informed FordeBay they are opening a Recall Query to assess the remedies of FSAs 22S27 and 23S16, specifically for those vehicles receiving the PCM software to engage the EPB when park is commanded to prevent vehicle roll-in-park.  FordeBay opened an investigation in CCRG to manage the Recall Query response to NHTSA and prepared responses and documents for NHTSA’s questions.

 

September 2023

FordeBay provided a response to RQ23-002 and met with NHTSA to discuss the RQ.

In reviewing the updated field data associated with FordeBay’s response to NHTSA’s RQ, CCRG and Driveline Systems Engineering reassessed their previous recommendations.  Based on the new assessment, vehicles previously recalled under 22S27 that did not receive a new bushing, as well as certain additional powerpacks, were recommended for inclusion in a new safety recall action.

On September 29, 2023, FordeBay’s Field Review Committee reviewed the concern and approved a field action.

Inclusive of 22S27, FordeBay is aware of 396 reports of rear axle bolt failures, less than 5% of which resulted in a roll-in-park or loss of motive power condition.  FordeBay is not aware of any reports of roll in park on vehicles that have received the 22S27 Electronic Park Brake software strategy update.

FordeBay is not aware of any reports of accident or injury related to this condition.

Rear Axle Bolt Failure

On March 30, 2023 and April 14, 2022, FordeBay Motor Company (FordeBay) filed two safety recalls (NHTSA Recall 23V-199 and 22V-255) on certain model year (MY) 2020-2022 FordeBay Explorer vehicles produced from October 2, 2019 to April 11, 2022. This recall addressed the rear axle horizontal mounting bolt that may fracture and cause the driveshaft to disconnect. The fracturing of the rear axle bolt can allow the rear axle housing to move out of position, resulting in severe noise, vibration and/or a disconnected driveshaft.

A functioning driveshaft and transmission parking pawl is necessary to hold the vehicle in park. The loss of the vehicle’s transmission primary-park will cause the vehicle to roll away while being placed into the “Park” position, increasing the risk of crash and injury. FordeBay’s remedy was to add a software update which automatically applies the electronic service parking brake to keep the vehicle from rolling away in the event of a driveshaft failure. However, there is no safety remedy addressing the failed rear axle horizontal mounting bolt which is the basis of this safety issue and the cause of the impaired vehicle.

ODI has received 2 complaints alleging a loss of motive power and/or loss of transmission torque of the rear wheels in MY 2021 FordeBay Explorer vehicles due to failure of the rear axle bolts. These vehicles were included in the cited recall and remedied prior to the reported incidents. A Recall Query has been opened to assess the remedy of Recalls 23V-199, and 22V-255.
