Loss of Electric Power Steering Assist

Summary

Ford Motor Company (Ford) is recalling certain model year 2011-2013 Ford Explorer vehicles manufactured May 17, 2010, through February 28, 2012. The affected vehicles may experience an intermittent connection in the electric power steering gear, which can cause a loss of the motor position sensor signal resulting in a shut down of the power steering assist.

 

Remedy

Ford will notify owners, and dealers will update the Power Steering Control Module (PSCM) software, free of charge. If a vehicle shows a history of a loss of motor position sensor signal when the vehicle is brought in for the recall remedy, its steering rack assembly will be replaced, free of charge. The recall began on July 23, 2014. Owners may contact Ford customer service at 1-800-392-3673. Ford’s number for this recall is 14S06.

PROGRAM TERMS

This safety recall involves reprogramming the Power Steering Control Module (PSCM), or replacing the steering gear if certain Diagnostic Trouble Codes (DTCs) are present.  If the steering gear is not replaced, the owner will be provided extended coverage of the steering gear under program code 15N01 (See Attachment II “LABOR ALLOWANCES”).  This extended coverage provides for one-time replacement of the steering gear if certain DTCs are present, for up to 10 years of service or 150,000 miles from the warranty start date of the vehicle, whichever occurs first.  If a vehicle has already exceeded the mileage limit, this program will last through December 31, 2015 for eligible owners.  The extended coverage is automatically transferred to subsequent owners.

 

AFFECTED VEHICLES

Certain 2011-2013 model year Explorer vehicles built at the Chicago Assembly Plant from Job #1 2011 through February 28, 2012.  Affected vehicles are identified in OASIS.  In addition, for a list of vehicles assigned to your dealership, visit https://web.fsavinlists.dealerconnection.com.  This information will be available on May 29, 2014.

 

REASON FOR THIS SAFETY RECALL

In some of the affected vehicles, the power steering system may revert to manual steering mode due to the Power Steering Control Module (PSCM) detecting a motor position sensor fault.  In manual steering mode there is still a mechanical linkage between the steering wheel and the road wheel, allowing steering control to be maintained.  If this condition should occur, a message will display in the Instrument Cluster Message Center, a chime will sound to inform the driver, and the steering effort may be greater, especially at low speeds, which may increase the risk of accident.

 

SERVICE ACTION

Before demonstrating or delivering any of the vehicles involved in this recall, dealers are to check the PSCM for Diagnostic Trouble Codes (DTCs).

  • If no loss of steering assist DTCs are present, reprogram the PSCM using IDS version 91.03 or higher. This closes Safety Recall 14S06, but Extended Coverage Program 15N01 remains open.
  • If any loss of steering assist DTCs are present and program 15N01 is open, replace the steering gear assembly. This closes both 14S06 and Extended Coverage Program 15N01.

The above services must be performed on all affected vehicles at no charge to the vehicle owner.

 

EXTENDED COVERAGE (15N01)

For lack of power steering assist concerns, dealers are to check the PSCM for DTCs.

  • If no loss of steering assist DTCs are present, the repair is not covered by 15N01. Proceed to normal diagnosis.
  • If any loss of steering assist DTCs are present and program 15N01 is open, replace the power steering gear assembly at no charge (parts and labor) to the vehicle owner.

NOTE:  To determine if the owner is eligible for extended coverage, enter the VIN in OASIS and check if program 15N01 is open.

 

OWNER NOTIFICATION MAILING SCHEDULE

Owner Letters are expected to be mailed the week of July 21, 2014.  Dealers should repair any affected vehicles that arrive at their dealerships, whether or not the customer has received a letter.

Owner Letters will be mailed the week of July 20, 2015 to notify customers of the extended coverage.

 

PLEASE NOTE:

Federal law requires dealers to complete this recall service before a new vehicle is delivered to the buyer or lessee.  Violation of this requirement by a dealer could result in a civil penalty of up to $7,000 per vehicle.  Correct all vehicles in your new vehicle inventory before delivery.

 

STOCK VEHICLES

Correct all affected units in your new vehicle inventory before delivery.

 

SOLD VEHICLES

  • Owners of affected vehicles will be directed to dealers for repairs.
  • Immediately contact any of your affected customers whose vehicles are not on your VIN list but are identified in OASIS. Give the customer a copy of the Owner Notification Letter (when available) and schedule a service date.
  • Correct other affected vehicles identified in OASIS which are brought to your dealership.

 

TITLE BRANDED / SALVAGED VEHICLES

Affected title branded and salvaged vehicles are eligible for this recall.

 

RELATED DAMAGE

If a related damage condition exists that you believe to be caused by the covered condition, call the Special Service Support Center to request approval prior to the repair of any related damage.  Requests for approval after completion of the repair will not be granted.  Ford Motor Company reserves the right to deny coverage for related damage in cases where the vehicle owner has not had this recall performed on a timely basis.  Additional related damage parts are subject to random selection for return to the Ford Warranty Parts Analysis Center (WPAC).

 

ADDITIONAL LABOR TIME

  • If a condition exists that requires additional labor to complete the repair, call the Special Service Support Center to request approval prior to performing any additional labor. Requests for approval after completion of the repair will not be granted.
  • If you encounter aftermarket equipment or modifications to the vehicle which might prevent the repair of the covered condition, call the Special Service Support Center.

 

OWNER REFUNDS

NOTE:  All refunds dated after July 1, 2015 must be claimed using Program Code 15N01.

  • This safety recall must still be performed, even if the owner has paid for a previous repair. Claiming a refund will not close the recall on the vehicle. 
  • Ford Motor Company is offering a refund for owner-paid repairs covered by this recall if the repair was performed prior to the date indicated in the reimbursement plan, which is posted with this bulletin. Owners are directed to seek reimbursement through authorized dealers or, at their option, directly through Ford Motor Company at P.O. Box 6251, Dearborn, MI 481216251.
  • Dealers are also authorized to refund owner-paid emergency repairs that were performed away from an authorized servicing dealer after the end date specified in the reimbursement plan. Non-covered repairs, or those judged by Ford to be excessive, will not be reimbursed.
  • Refunds will only be provided for the cost associated with steering gear replacement for loss of steering assist.

 

RENTAL VEHICLES

If a customer’s vehicle requires the replacement of the steering gear and it is necessary to order parts, Ford Motor Company will pay for one day of vehicle rental.  Follow Extended Service Plan (ESP) guidelines for dollar amounts.  The daily rate can include applicable taxes but are not allowed to exceed the stated daily rate.  Rentals will only be reimbursed for the day the vehicle is at the dealership for part replacement.  Prior approval for more than one rental day is required from the Special Service Support Center (1-800-325-5621).  The parts order must be an emergency order (unit down) if the order is placed between 3:00 PM and 7:00 PM (your local time zone) to guarantee the shortest delivery time.

CERTAIN 2011-2013 MODEL YEAR EXPLORER VEHICLES — REPROGRAM POWER STEERING CONTROL MODULE AND POWER STEERING GEAR EXTENDED COVERAGE

 

OVERVIEW

In some of the affected vehicles, the power steering system may revert to manual steering mode due to the Power Steering Control Module (PSCM) detecting a motor position sensor failure. In manual steering mode there is still a mechanical linkage between the steering wheel and the road wheels, allowing steering control to be maintained. If this condition should occur, a message will display in the Instrument Cluster Message Center, a chime will sound to inform the driver, and the steering effort may be greater, especially at low speeds.

The purpose of this repair is to update the PSCM to prevent loss of steering assist while driving due to a motor position sensor fault or replace the steering gear if the PSCM has certain Diagnostic Trouble Codes (DTCs). Effective immediately, if the steering gear needs to be replaced, it must be claimed under extended coverage program code 15N01. Replacing the steering gear under program code 15N01 will automatically close Safety Recall 14S06.

 

SERVICE PROCEDURE

1. Connect IDS and check for DTCs in the PSCM.

  • No – If 14S06 is open in OASIS, proceed to step 2. If 14S06 is not open in OASIS, the repair is not covered under this program. Proceed to normal diagnosis.

NOTE: Reprogramming of the PSCM is not required if the steering gear is replaced. Therefore, replacing a steering gear under program code 15N01 will automatically close Safety Recall 14S06.

2. Reprogram the PSCM using IDS version 91.03 or higher.

NOTE: Calibration files may be obtained at www.motorcraft.com.

NOTE: Follow the IDS on-screen instructions to complete the reprogramming procedure.

 

Important Information for Module Programming

NOTE: When programming or reprogramming a module, use the following basic checks to ensure programming completes without errors.

  • Make sure the 12V battery is fully charged before carrying out the programming steps and connect IDS/scan tool to a power source.
  • Inspect Vehicle Communication Module (VCM) and cables for any damage. Make sure scan tool connections are not interrupted during programming.
  • A hardwired connection is strongly recommended.
  • Turn off all unnecessary accessories (radio, heated/cooled seats, headlamps, interior lamps, HVAC system, etc.) and close doors.
  • Follow all scan tool on-screen instructions carefully.
  • Disable IDS/scan tool sleep mode, screensaver, hibernation modes.
  • Create all sessions Key On Engine Off (KOEO). Starting the vehicle before creating a session will cause errors within the programming inhale process.

 

Recovering a module when programming has resulted in a blank module:

NEVER DELETE THE ORIGINAL SESSION!

1. Obtain the original IDS that was used when the programming error occurred during Module Reprogramming (MR) or Programmable Module Installation (PMI).

2. Disconnect the VCM from the Data Link Connector (DLC) and the IDS.

3. Reconnect the VCM to IDS and then connect to the DLC. Once reconnected, the VCM icon should appear in the corner of the IDS screen. If it does not, troubleshoot the IDS to VCM connection.

4. Locate the ORIGINAL vehicle session when programming failed. This should be the last session used in most cases. If not, use the session created on the date that the programming failed.

NOTE: If the original session is not listed in the previous session list, click the ”Recycle Bin” icon at the lower right of the previous session screen. This loads any deleted sessions and allows you to look through them. Double-click the session to restore it.

5. Once the session is loaded, the failed process should resume automatically.
f. If programming does not resume automatically, proceed to the Module Programming menu and select the previously attempted process, PMI or MR.

6. Follow all on-screen prompts/instructions.

7. Near the end of programming, the IDS prompts you to select certain parameters. It is important to make a selection for ALL parameters listed. If the correct selection is already highlighted, you must still choose that selection before clicking the “Tick” mark to complete the configuration.

8. The last screen on the IDS may list additional steps required to complete the programming process. Make sure all applicable steps listed on the screen are followed in order.

Q1. What is the problem? 

Ford previously voluntarily recalled certain 2011-2013 model year Explorer vehicles to address concerns relating to power steering operation.  In some of the affected vehicles, the power steering system may revert to manual steering mode due to an Electric Power Steering (EPS) system fault related to the motor position sensor.  In manual steering mode there is still a mechanical linkage between the steering wheel and the road wheels, allowing steering control to be maintained.  If this condition should occur, a message will display in the instrument cluster message center and a chime will sound to inform the driver.  Loss of power steering assist would require greater steering effort, especially at lower speeds, which may increase the risk of accident.

 

Q2. What does the software update do?

The software update may extend the time steering assist is maintained if a motor position sensor fault occurs.

 

Q3. What if a customer experiences loss of steering assist after the module has been reprogrammed?

The module was reprogrammed to prevent sudden loss of steering assist while driving if a motor position sensor fault occurs. Any subsequent loss of assist experienced after completion of module reprogramming may be eligible for extended coverage under program code 15N01.  Check OASIS to determine eligibility.

Electric power steering failure

On May 27, 2014 and amended June 2, 2014, Ford Motor Company (Ford) submitted a Defect Information Report (DIR) to NHTSA describing a safety defect that may result in a sudden loss of power steering assist in approximately 179,027 model year (MY) 2011 through 2013 Ford Explorer vehicles equipped with electric power assisted steering (EPAS), including 82,328 MY 2011 Explorers that are the subject of PE12-017. Ford has assigned the recall number 14S06. The NHTSA recall number is 14V-286. Ford’s DIR indicates that loss of power steering assist while driving would require higher steering effort at lower vehicle speeds, which may result in an increased risk of a crash.

Sudden loss of power steering assist while driving can occur in the subject vehicles if the system loses electrical power or whenever the system detects a fault that requires it to enter fail-safe mode, which removes power from the EPAS motor and defaults to manual steering. Ford’s DIR indicates that the majority of steering assist failures in the recalled vehicles have been caused by an intermittent electrical connection in the Power Steering Control Module (PSCM) that can lead to a loss of the motor position sensor signal. Ford’s remedy instructs dealers to check the PSCM for diagnostic trouble codes (DTC). If upon initial inspection a loss of steering assist DTC is present, dealers will replace the steering gear at no charge to the owner and update the PSCM with revised software. Following detection of a motor position sensor signal fault, the revised software will provide audible and visual warnings to the driver that a power steering system fault has been detected and will maintain steering assist for the remainder of that drive cycle. If no DTC is present, dealers will reprogram the PSCM with the revised software.

Analysis of information from all sources identified a total of 969 complaints and 4,059 warranty claims related to loss of power steering assist while driving in the MY 2011 through 2012 Explorer vehicles, resulting in a complaint rate of 5.4 incidents per thousand vehicles and a warranty claim rate of 2.3 percent. ODI identified 15 crashes with evidence indicating loss of power steering assist may have been a factor. All 15 crashes involved low-speed impacts with roadside objects during turning maneuvers, resulting in minor vehicle damage or no damage. In one incident that occurred in a low-speed curve, the driver attempted to brake in response to the sudden increase in steering effort and inadvertently applied the accelerator pedal instead. This caused the vehicle to run off the side of the road and strike a wooden pole at approximately 15 mph. Another incident occurred when the driver was unable to to negotiate a left-turn at an intersection and ran off the road into a shallow ditch, resulting in moderate front-end damage to the vehicle and minor injuries to the driver that did not require medical treatment.

This investigation is closed. All ODI complaints associated with this closing resume are listed in the Attachment 1.
